DOCUMENTS FOR STUDENTS

- This is a PDF of the Fieldwork Handbook and has all of the information needed for fieldwork. Please review the entire document to see what is expected of you and your fieldwork (host) teacher during your placement. After reviewing the PDF, follow the link to the eForm to acknowledge you have read it.
- Memorandum of Understanding/Professional Expectations for Students – Read & Agree (eForm)
- Review this document to understand more fully what the professional expectations and logistical requirements are for your fieldwork placement. After you have reviewed this document, click on the eForm to acknowledge that you have read and understand these requirements and expectations.
- Fieldwork Log – Print, Scan/Photo & Upload Below
- Print out the fieldwork log to record your hours. This log must be signed by both your fieldwork (host) teacher and your Pratt faculty member and at the end of the semester, a completed log must be scanned and uploaded using the upload button below. No grades are completed until the final fieldwork log is received.
DOCUMENTS FOR FIELDWORK (HOST) TEACHERS
Your fieldwork teacher is required to complete the following online documents. Use the links below to download copies of the documents for your fieldwork teacher to read, print, sign and/or upload as appropriate. You can then upload the documents for them using the UPLOAD BUTTON.
- Fieldwork Handbook – View Handbook (PDF) & Confirm Read (eForm)
- Point your fieldwork (host) teacher to the following documents in the Fieldwork Handbook: Fieldwork Contract (p.9) and Glossary (pp. 7-8). After you and your host teacher have reviewed the PDF of the Fieldwork Handbook, particularly the pages identified above, follow the link to the eForm to acknowledge that you have reviewed it with your cooperating teacher.
- Fieldwork Faculty Contract– Print, Sign, Scan/Photo & Upload
- Print or photocopy the fieldwork contract (p. 9) for your fieldwork (host) teacher. Your host teacher is required to review and sign the document to acknowledge that they understand the requirements of your student teaching placement. Once the document has been signed by your host teacher, scan/photo the document and upload it to the ADE eForm site using THE UPLOAD BUTTON.
