As a student teacher you are required to complete the following documents and upload or confirm that you have read them on our ADE eForm platform. Use the links to download copies of the documents to read, print, sign and/or upload as appropriate.
DOCUMENTS FOR STUDENTS
- Student Teaching Handbook – View Handbook (PDF) & Confirm Read (eForm)
- Please review the entire document to see what is expected of you and your cooperating teacher during your student teaching placement. After reviewing the PDF of the Student Teaching Handbook, follow the link to the eForm to acknowledge you have read it.

- Download an ADE Lesson Planning Template.
- Student Teaching Log – Print, Scan/Photo & Upload Below
- Print out the student teaching log as necessary to record your student teaching hours. This log must be signed by your cooperating teacher weekly while at your placement. The completed log must be signed by your Pratt faculty supervisor and at the end of the semester, a completed log must be scanned and uploaded using the upload button below.

DOCUMENTS FOR COOPERATING TEACHERS
Student teachers are responsible for reviewing documents with their cooperating teachers and obtaining and uploading any necessary signatures. Use the links below to download copies of the documents for your cooperating teacher to read, print, sign and/or upload as appropriate. You can then upload the documents for them at any time on the ADE eForm site using the UPLOAD BUTTON BELOW.

- After you and your cooperating teacher have reviewed the PDF of the Student Teaching Handbook, particularly the section titled: Information for Cooperating Teachers, follow the link to the eForm to acknowledge that you have reviewed it with your cooperating teacher.
- Collaborative Partnership Agreement – Print, Sign, Scan/Photo & Upload Below
- Your cooperating teacher is required to review and sign the document to acknowledge that they understand the requirements of your student teaching placement. Once the document has been signed by your cooperating teacher, scan/photo the document and upload it to the ADE eForm site using our UPLOAD BUTTON.
